We’ve streamlined the care of patients with chronic or long-term medical conditions by allowing them to have multiple, concurrent care plans and more than one care manager. And now patient lists created from the Einstein Analytics for Health Cloud app can be sent to and used in the Health Cloud console. To make your life easier as an admin, we’ve packaged up three new Lightning components that let you create patient communities in no time at all.

Create Concurrent Care Plans
Managing care for patients with multiple conditions just got a lot easier. Care coordinators can create concurrent care plans to view and manage patient problems, goals, and tasks in a condensed view in the console.
Use Health Cloud Empower Lightning Components to Build a Patient Community
Health Cloud Empower Lightning components let you easily build a patient community in no time at all. Use the Health Cloud Empower components and the Customer Service (Napili) template to quickly roll out a patient community with easy customization using Community Builder.
Send a Patient List from the Analytics App to Health Cloud
Integrated Analytics lists let you use the intuitive Einstein Analytics for Health Cloud app to create a patient list and export it to the Health Cloud console. In the console, care coordinators can use these highly segmented lists to more effectively manage patients.
Control the Sort Order of Items in Care Plans
Care coordinators can now specify the order in which problems, goals, and tasks appear in a care plan so that the most critical items are listed first.
Create Care Plans That Fit Your Patients
Create custom care plan record types to give care coordinators flexibility in tracking and managing their patients by using different types of care plans.
Get Timeline Details at Your Fingertips
Give your users more information about patient timeline events by adding extra information to the hover text. And console users can now click an event to open the related record in another tab so that all the details of the event are available.
Procedures and Procedure Requests Added to the Timeline
Now care coordinators can see a patient’s medical procedures and requests for medical procedures on the timeline.
Delete Patient Lists
Now care coordinators can keep their patient lists neat and manageable by deleting obsolete and unwanted lists. Care coordinators can delete the custom patient lists that they created. And if they have the appropriate permissions, they can also delete patient lists created by others.
Archived Tasks Are Available in Health Cloud Console
Tracking a patient’s care can be an ongoing process that spans many years. To help care coordinators with long-term planning and management, we’ve given them full access to view and edit archived care plan tasks. You can see and manage archived tasks on the patient’s timeline, in the care plan, and on the Today page.
Assign a Permission Set License
Control access to Health Cloud by assigning the Health Cloud permission set license to each Health Cloud console user. Starting with the Summer ’17 release, you must assign the Health Cloud permission set license to each user. This requirement includes Health Cloud packages installed in a production or sandbox org after the Summer ’17 release. If you installed the Health Cloud managed package before the Summer ’17 release, this step is optional until further notice.
Add Visualforce Page Access to Console Users
Orgs created before Summer ’17 must add HcFilterLogicValidationPage access to the permission set of users who create patient lists in the Health Cloud console.
