Complete a Health Assessment on Someone’s Behalf

An assessment survey is a valuable way to efficiently gather information about the health of the people whose care you manage. But what if you’re performing a phone screening or managing someone who doesn’t have access to a community? Now you can open the assessment and complete the questions on behalf of the patient or member you’re managing.

Where: This change applies to Lightning Experience, Salesforce Classic, and all versions of the Salesforce app in Enterprise, Performance, and Unlimited edition where Health Cloud is enabled.

How: On the Available tab, select Complete Assessment, open the assessment survey, and take the assessment on behalf of the person you’re speaking with. Your name is saved as the submitter of the assessment.
